Sarah was the daughter of Governor Isaac Shelby and Susannah Hart. She married Dr. Ephraim McDowell in 1802 and they were the parents of nine children with only five living to adulthood.

Sarah and Ephraim were both originally buried in the Shelby Family Burying Ground in Lincoln County, Kentucky. Their remains were reinterred in 1879 in McDowell Park, which was created and dedicated in Dr. McDowell's honor by the Kentucky Medical Society on December 13th and 14th, 1879.

Four of their children remain interred in the Shelby Family Burying Ground in Lincoln County.
