The son of John Hill Smith and Mary Cary Ambler Smith attained his Medical Degree from the University of Virginia in 1834. He practiced in Richmond. He married Jane McDonald Briggs in 1836 and moved to Dinwiddie County. Jane died in 1842. In 1844 he married Anne Gilliam Anderson. He practiced in Dinwiddie and farmed until the Provisional Army of the Confederate States appointed him Surgeon at Chimborazo Hospital in Richmond on 16 October 1861. After receiving a favorable report from the Surgeon General, he was made Surgeon in Charge of Chimborazo Hospital Number Three. He remained in control of that until he received his parole in April of 1865.

After the War he practiced in Richmond. He represented Dinwiddie County in the Virginia House of Delegates from 1877 to 1880, when President Hayes appointed him U.S. Consul to Naples. In 1882 he returned from Italy on account of his health.
