FATAL ACCIDENT NO. 42 — June 19, 1908 - Julius Pizza, Franklin Mine.
An inquest was held on this date (June 20) on the body of Julius Pizza, who died on the 19th day of June from injuries received on June 17th at the 9th level, No. 5 shaft, Franklin Mine.
D. R. Orella sworn as interpreter for Barrolia Olian gelo said: “I am employed by the Franklin Co. , as a company account man. I know deceased, he worked with me. When he got hurt, he was holding the drill, while I was striking, that was at the 9th level, No. 5 shaft, on the 17th day of June between one and one-thirty o'clock. We were block holing - drilling a hole and the rock split in two, and the half of it rolled down the stope carrying deceased with it. He died last night. The rock was up about ten feet, did not see any rusty head. ”
Peter Dibene, another witness, corroborated the above testimony adding: “I tested it with the bar, but it would not break. I did not notice the rusty head, we thought it was perfectly safe as we tested it.

Verdict: "We, the jury, upon our oaths, do say, that the said Julius Pizza, came to his death, on the 19th day of June, A.D., 1908, from injuries received to the pelvis and bladder, on the 17th day of June, 1908, at the 9th level, No. 5 shaft, Franklin Mine by the splitting of a rock while block holing, and part of the rock rolling down the stope carrying him with it."
