ACCIDENT NO. 21- (Two Deaths) -April 28, 1911, Thomas Dodge and Joseph Kestner, No. 16 Shaft, Osceola Amygdaloid, C. & H. Thomas Dodge, an Englishman and Joseph Kestner, an Austrian, both miners, came to their death on this day by a fall of rock in the 3rd level, North of No. 16 Shaft, Osceola Amygdaloid, C. & H.

At the inquest held over their bodies by Justice Fisher, the following witnesses testified:

Antony Binando: "I was working at the place of the accident which happened about eleven o'clock this morning. We were out in the drift with the empty car waiting for the report of the blast. Dodge and Kestner were there too. As soon as the report was heard we went in. We got the car half full when we heard a crash. We looked toward where Dodge and Kestner were and their lights were out. I ran back to where the other party of miners were and told them the miners were buried under poor rock. We went in and got them out. They could not have been more than 15 or 20 feet from the level. They were not barring. They were just looking around after their blast and speaking about the copper."

Edw. Modrok: "I can only say in addition to above witnesses' testimony, that I saw the place after the ground fell and it did not look bad. They looked the hanging over yesterday. I saw them doing it. I should judge the rock fell about 12 or 15 feet."

Mike Segola: "I am a trammer at the 3rd level, No. 16 shaft and corroborate the testimony of Antony Binando."

Verdict: "We find, that the said, Thos. Dodge and Joseph Kestner came to their death by a fall of rock in the 3rd level, North of No. 16 Shaft, Osceola Amygdaloid, Calumet & Hecla Mine on this 28th day of April, 1911."
