Her parents were William and Jane (Watt) Phelps. This is shown, in part, by Jane's probate record, in which she makes a bequest to her grandsons, William and Edward McIntosh (those are the names of Sarah Elizabeth's known children).

It appears that the marker in this cemetery is a cenotaph. On an indexo of Allen County obits I found one (partial, apparently partially illegible) regarding the death of Elizabeth (as Sarah was known) in Strawberry Valley, Cal., and noting that she is the daughter of Jane Phelps and wife of Alexander [no surname given]. I will attach that screenshot to Sarah's memorial page, as it clarifies a lot. Alexander was later buried in a family plot in Plumas County, CA, as was his son Edward Phelps McIntosh. Edward's obit mentions the place as "Strawberry" so apparently the same place where his mother had died a month after Edward was born.

According to an October 1860 partial obit (apparently hard to read) on a Fort Wayne historical website, Sarah was the daughter of "Mrs. Jane Phelps" and died in Strawberry Valley, California. That is conssitent with the known facts, including a marriage record dated 1858, and an extract from a family Bible that is posted on Ancestry.com.
