The unikat (original copy) version of the image is easier to read. It gives his parent's names as Majer and Estera.

A partial translation of the record indicates that he left his wife Perla, and "children, four of which are" Esterka, Chaie (i.e. Chaya Sura), Baruch and Dawid.

The Akta doesn't give his patronymic, unfortunately.

It appears that Shmerel and Elka (his first wife) had an infant son named David who died as a child. Shmerel's son David (with his second wife, Perla Troppe) may well have been named after him.
