On her own death certificate, her name is Chia Rachel Litofsky and her father's name is Israel Gabai (handwritten), which matches the Hebrew first name of her father on her grave (ישראל) and is a close match to Simon's death certificate.

He granddaughter who was named in her memory (Jennie's daughter Ida), went by Chia Rachel, Ida Ruth, Ida Rose, and Rose Ida.

Because of this:

(1) it seems Rachel had a second husband after Yom Tov Lipman Molofsky (after the father of her children) but was buried using the name she used for most of her life, and which she shared with her children. (2) it seems Gelbau is the most reliable version we have for her maiden name, as the handwritten similar name is harder to make out.
