It's from the meeting records of the Pasquotank Monthly Meeting, as reprinted in "Encyclopedia of American Quaker Genealogy, Vol. I," p. 120.

Though I should note that as of the moment, I'm only finding "Edward and Susannah Tadlock," not further details on them. I'll keep browsing to see if I find Susannah's maiden name or other details.
