Accident 16. - Occurred January 5, 1894, at the 30th level in No. 10 shaft, South Hecla branch of the Calumet and Hecla mine.
Andrew Ervin was assisting to put in some dividing timbers to partition off the hoisting shaft from the ladderway. While a piece of timber was being hoisted, Ervin went to pry it from the foot-wall rock, and by an oversight he went hoisted. As soon as he loosened the timber from the foot-wall rock, it swung toward him and jammed his head between the timbers.
He died in about one hour afterwards.
An inquest was held before Justice Sowden, and the jury returned a verdict of accidental death.

Onnettomuus nro 16. - 5.1.1894, Calumet & Hecla -yhtiön South Hecla -kaivoksen tasolla 30 kuilussa nro 10.
Andrew Ervin avusti puurakenteiden asentamisessa, jotta nostokuilu saatiin erotetua tikasnoususta. Kun parrua nostettiin, Ervin meni irrottamaan sitä kallioseinämästä ja epähuomiossa joutui nostetuksi mukana. Kun parru irtosi seinämästä se heilahti häntä kohti ja rusensi hänne päänsä parrujen väliin.
Hän kuoli noin tunnin kuluttua tapaturmasta.
Kuolinsyyntutkinnan johti tuomari Sowden ja valamiehistö totesi kuoleman tapahtuneen tapaturmaisesti.
