ACCIDENT NO. 15— April 23rd, A.D. 1906, Hecla Branch of Calumet and Hecla Mine, Isaac Mode(Modig). This accident occurred at the 32nd level north of No. 3 shaft, Hecla Branch of the Calumet and Hecla Mine by a fall of vein rock.

Inquest was held by Coroner Fisher.

Edward Andrews testified as follows: "I work in No. 3 shaft at the 32nd level north side of shaft. I knew the deceased Isaac Mode(Modig). He was my partner, he was working with me at the time of the accident. About half-past eleven this morning we were blasting. We blasted one round of five holes and when the five went off we went back again, and charged up the other four. We had nine in all. Eight holes and one block hole. We charged them all up and had them all snuffed. We then pulled back a long plank about 16 feet, Isaac had the front end and I was behind. When we came back I was in front and Isaac was behind, and I heard something drop. I called and asked him if he was alright, but got no answer. I turned and went to look for him and saw that he was killed. The vein rock fell away, a piece about four feet long and eighteen inches thick. The ground looked allright. There was a loose piece. We tried it in the morning with a bar but it would not give, but that was not the piece that came away there was a good hanging and a good back, everything looked safe, the rock that fell was probably loosened up when we blasted."

Verdict of the Jury: "We the Jury, upon our oaths, do say that the said Isaac Mode(Modig) came to his death by a fall of vein rock in the 32nd level north of No. 3 shaft, Hecla Branch of the Calumet and Hecla Mine on the 23rd day of April, A.D. 1906."
