CHURCH - In Otselic, N.Y., April 14, 1890, Samuel C. Church, ages 72 years, 2 months and 28 days. In early manhood he made a profession of religion, was baptized and united with the Otselic Church, and so continued through life. He served his country faithfully, and lived to return and spend many years with his family, entertaining the ministers in his hospitable home, and greatly delighting to attend our public gatherings. Last June, he was taken severely sick with heart-disease, and at times was considered dangerous, but he lingered many months, and at last passed away trusting in the Saviour" L.R.S.

From NYS Adj.-Gen. report 1903: "Church, Samuel - Age, 44 years. Enlisted, August 4, 1862, at Otselic, [Chenango County, NY,] to serve three years; mustered in as sergeant, Co. G, [114 NYSV,] August 14, 1862; discharged for disability, March 28, 1863, at Marine Hospital, New Orleans, La."
Certificate of Disability for Discharge states: "Samuel Church, a Private of Captain H.S. Wheeler, Infantry Company, of the 114th New York regiment of United States Volunteers was enlisted by J.F. Thompson, the Regiment of ____ at Otselic, N.Y. on the fourth day of August 1862, to serve three years; he was born in Chenango County in the State of New York is forty four years of age, five feet eight 3/4 inches high. Dark complexion, black eyes, brown hair, and by occupation when enlisted a farmer. During the last two months said solider has been unfit for duty 60 days. -- [handwritten:] he shot himself through the hand about the 18th January, accidentally while shooting at a hog. U.S. Gen. & Marine Hospital, New Orleans, La. Feb 15, 1863, Jacob Bockee Surgeon in Charge.
I certify that I have carefully examined the said Samuel Church.... pistol ball passing directly through the hand and permanently diabling it. The wound was not received in the line of his duty. Jacob Bockee. Surgeon of Vol. Discharged, this 26 day of February 1863 at Marine Hospital New Orleans."
