I think it is improbable that Sterna, the granddaughter of Chaya Lubarski Zaslavski (Horodenker), married that same grandmother's son from another marriage. This does not make sense.
As posted, Sterna was the granddaughter of Chaya Lubarski Zaslavski (Horodenker) and Chaya's 1st husband, Rabbi Nachman "Chayalas" Lubarski.
Sterna's alleged husband, Yechiel, was the son of Chaya Lubarski Zaslavski (Horodenker) and Chaya's 2nd husband, Rabbi Aharon Klutzkar Zaslavski of Kremenchug.
If Sterna's father was born in 1815 (when Chaya was only age 14), then Chaya's first son, Rabbi Nachman, was only 10 years old in 1825, the year that Sterna and Rabbi Yechiel were reportedly born.
None of this makes sense halachically.
