- Jeff was born in New York and raised in Plainview. His exact town of birth has not been cited in the press coverage about him, nor has a New York birth index been made available.
- Although his Ohio death certificate says that he died in Ravenna, he was only declared dead there. His actual death was instantaneous and occurred in an access road by the center of the Kent State campus.
