The cemetery is located in Ellsworth, Pierce County, Wisconsin.

A log church was built in 1863 on this site. The predominately Irish Roman Catholic parish was named Our Lady of Perpetual Help. A new church building was built about a mile south on what is now known as Co. Rd. DD in 1884, with a new cemetery established at this site. The log building at the old site was used for storage for a period of time and was eventually torn down in the early 1900's.

The Old Clayfield Cemetery is minimally maintained, and is steep and rolling. It is located on a heavily wooded hillside and is poorly marked. Few people know of the location, noted only by a paver block retaining wall along the road and a chain link fence. The cemetery records have been lost, and some of the markers have been removed.
