Per original death certificate (from Louisiana State Archives), excerpt: "Be it Remembered, That on the day to-wit the Fourteenth of September in the year of our Lord One Thousand Nine Hundred & Fifteen and the One Hundred and 40 of the independence of the United States of America, before me W. T. O’REILLY, M. D., Chairman Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages in and for the Parish of Orleans, personally appeared A. Wolf an undertaker native of this city residing at No. 527 Elysian Fields Avenue who hereby declares that Mrs. Adeline Henerichs (White) a native of Mississippi, aged 63 years departed this life this day (14 Sept 1915) at the Charity Hospital in this city. Cause of death Diabetic Gangrene Gas Infection. Certificate of Dr. B. Tillery. Deceased was married and remains shipped to Mandeville, La."
