For some reason I made a mistake with the year and thought its 5678, 100 years ago. But it's not. His passing away was 1919, 5679. April 28 1919, four months **After** WWI.
His daughter Chaya's tombstone was recently found and on it was listed that she passed away in Adar II of 5679 and that he was still alive (Nero Yair, may his candle continue lighting) which meant that the passing of Rabbi Yitzhok in 5678 couldn't be true. My father, שיחי' לאורך ימים ושנים בטוב ובנעימים proved from this that it was a mistake.

On 28 of Nisan 13 to the Omer is the day Bnei Yisroel entered Eretz Yisroel under Yehosua.
A side note: Hebron had been liberated by the British from the Turks on 19 Kislev that year, and Jerusalem surrendered on the first night of Hanukka.

A convention on the future of Eastern Europe after the war (WWI) had closed that day.
UPDATE: It turns out that the militias fighting in Lvov (Lemberg) at the end of the war, and also attacking civilians, Rabbi Yitzhak was wounded by a bullet. He was in the hospital for a year, and finally passed away at the age of 53.
He is burried in the old Lemberg cemetery. It may be a cemetery that was demolished by the Nazis and turned into a market.

The Yizkor book says (as a joke) that the Jews of Korczyn were so rich, in the first years that they were worried that they wouldn't be able to give Matonos Laevyonim to the poor on Purim.
Korczyna Jewry was demolished on Erev Rosh Hodesh Elul 5703 (April 12, 1942, a fatal day for many towns of Jews under German occupation)

Update: Searching for 'Lemberg 1916' I found interesting images of the commander of a Austro-Hungarian Howetzer gun unit fighting alongside the Turkish forces in Gaza and buried in Jerusalem was from Lvov - Lemberg: Here's a picture of the Austro-Hungarian army leaving Jerusalem Erev Hanukah 676, http://riowang.blogspot.co.il/2012/05/lemberg-fifth-grave-of-hero-o... The picture shows several Jews leaving the city with them, one is wearing a Shtreimel. Rabbi Yitzchok's brother in law Menachem Mendel Rubin was the head of the Hassidic Jews (Kolel Galicia) here, so perhaps the Jews at the head of the civilian procession is him!

הפניצילין נתגלה רק בזמן המלחמה והיה נפוץ רק בידי כוחות הברית (אנגליה, צרפת, וארה"ב) ולא היה בידי הכחות במזרח (הממלכה האוסטרו הונגרית, הצאר - ואחריהם הקומוניסטים, וכמובן העותמנים).
