The cemetery is located at 8660 State Route 66. The cemetery was established in 1868 as Forest Hill Union Cemetery via a joint purchase of the land by Washington Township and the city of Piqua. In 1904, the Forest Hill Cemetery board of trustees purchased what is now the St. Joseph section in the far northeast area of the current cemetery. That section had previously been owned and operated by the Catholic Church.

Forest Hill Union Cemetery goes back to 1868 when officials from Washington Township and the City of Piqua purchased nearly 95 acres from Jacob Schmidlapp for a sum of $14,512.50. It held its first burial the same year, that of Sarah Barnett. In 1880, the Township and City purchased additional land from Jacob Schmidlapp.

In 1904, the Forest Hill Cemetery board of trustees by purchased the present-day St. Joseph section located in the far northeast are of the cemetery. At the time it was owned and operated by the Catholic Church.

As of today there are an estimated 29,500 burials and over 27,000 monuments.
