Awarded by the Ontario Government to members of the Ontario Provincial Police for 20 years service.
The qualifications are:
- The Provincial Police Long Service and Good Conduct Medal was awarded to those members of the Force who were of irreproachable character, and who had served in the Force not less than 20 years, the last 10 of which was with good conduct and satisfactory service.
- Medal awards were made to any authorized regular members of any municipal police force absorbed into the Ontario Provincial Police as a result of a policing contract between the Commissioner and such municipality. In such cases, regular municipal police service, which must have been continuous up to the time of absorption, were counted towards qualifying service for the award.
- War service in the armed forces of Canada could be counted towards qualifying for the award in cases of those who were members of the Force immediately prior to joining the armed forces and who rejoined the Force within three months of their honourable discharge from the armed forces.
- Recommendations for the medal awards were to be made by the Officer commanding the district, or administrative branch in which the member of the Force in question was serving.

The recipients of this medal would also receive the Police Exemplary Service Medal from the Governor General for the same 20 years of service

- 25 Years Service - 1 Silver Trillium device
- 30 Years Service - 2 Silver Trillium devices
- 35 Years Service - 1 Gilt Trillium device
- 40 Years Service - 2 Gilt Trillium devices
Description
A circular, silver medal, 38 mm in diameter made of stirling silver.
Obverse
The observe features the Coat of Arms of the Province of Ontario part of which is the motto VT INCEPIT FIDELIS SIC PERMANET with the word ONTARIO underneath the crest.
Reverse
The words ONTARIO PROVINCIAL POLICE are surrounded by the words FOR LONG SERVICE AND GOOD CONDUCT and at the base of the medal are two crossed maple leaves.
Date of establishment
The Order was created in 1949 by the Ontario Legislature.
Naming
On the rim of the medal is engraved the rank or regimental number, initials and name of the recipient. A scroll type mounting is used.
