He died at Harper Hospital in Detroit. His cause of death is listed as exophthalmic goiter, (Grave's Disease). The informant was his brother, Herbert, who incorrectly gave Lewis' place of birth as Canada instead of Indiana, and did not know his date of birth. Month and year of birth are from the 1900 US Census.
