Provo Woman Dies. Special to The Tribune.
PROVO July 11 — Mrs. Anna Hardy Madsen wife of the chief deputy fish and game warden, David H. Madsen, died at the family home, 748 West Center Street, at 3:30 o'clock this morning of Bright's disease, from which she had suffered for the last five months. She was thirty-one years of age and was born in Provo and lived here all her life. She was the daughter of James and Elizabeth S. Hardy.
Besides her husband she is survived by a mother, two sisters, Mrs. Phebe Jones of Provo and Mrs. Pollie Russell of St. George, and four brothers, Harold H. and Albert Hardy of Garfield and Thomas S. and George W. Hardy of Provo.

Annie Hardy Madsen is the daughter of James Hardy and Elizabeth Smith Hardy.
She is the granddaughter to William Manning Hardy & Mary Ann Hyde Hardy and James Smith & Elizabeth Turner Smith.
She is the sister to Sarah Elizabeth, Albert Smith, George Willard, and Thomas Smith.
She married David Heber Madsen on October 9, 1900. She died two years later. There is no record of any children.
